East Side Nursing and Rehabilitation's Restorative Rehabilitation Program is available 5 days a week providing the optimal therapy necessary for recovery. The therapy team works closely with our nurses, physicians and families to coordinate care allowing you to make ideal progress.
When appropriate our therapists offer home assessments prior to discharge. This is sometimes necessary to promote the confidence a resident or their family may need before returning home.

Upon reaching your goal in rehabilitation, the knowledge and experience of East Side Nursing and Rehabilitation's Discharge Planning Team eases your transition back to your previous living arrangements. They will meet with you and your family to answer any questions and safety concerns regarding your return home. During this meeting, the Team’s main objective is to provide you with the means of making your return home an easy transition. They will assist you in obtaining the necessary equipment, make referrals for home care and support services as well as offer suggestions for any changes needed in your home.
